This case report describes the treatment of a 15-year-old girl with a dental and skeletal Class II malocclusion and missing both upper lateral incisors. Also patient presented both lower second premolars impacted. The first stage of the orthodontic treatment included the extrusion of the impacted premolars using the biomechanics of .019”x.025” TMA cantilevers anchored by 2 miniscrews 2 x 12 mm placed at buccal shelf (BS) area. After 6 months, both teeth were in position. Extruding impacted teeth at the mandibular premolar area using conventional mechanics may require auxiliary device such as lingual holding arch, removable appliances or even bonded brackets. The application of miniscrews at the BS area during tooth extrusion showed to be simple, stable, had no dental side effect.
