Abstract

An 11-year-old girl presented with a bimaxillary Class I malocclusion, missing space for the upper left canine, posterior crossbite, and aggravated by a failure of eruption of the lower second molars. At an early phase of the treatment, rapid palatal expansion appliance and lip bumper therapy were used to gain arch length. Since the patient had negative tooth size/arch length discrepancy, for the corrective treatment, four bicuspid extractions were planned. To correct the second molars position, two miniscrews 1.5 × 10-mm were placed into retromolar area and elastic chains were used to correct their position. After 36 months of treatment the left canine was placed in the correct position, lower molars were uprighted, Class I molar and canine relationships and satisfactory inclination of upper and lower incisors were achieved, as well as good overbite and overjet. Due to the severe anterior and posterior discrepancies, patient's profile was maintained and the overall esthetics improved. It was possible to conclude that miniscrews are a powerful tool to assist challenging movements such as molar uprighting in situations that anterior anchorage is critical.
